**To make it show Name,Phone and address make sure you have that fields in your custom fields for your customer entity .
and then Can you try put this in your display format>>
Customer Name:[Name]<br/>Phone: [Phone]<br/>Address:[Address]<br/>

Those tags are v5 only. V4 does not support that

They work with v5. You won t get this to work in v4. You should upgrade to v5. V4 will never get anymore updates.

Can you show a screenshot where you saw It? Maybe you saw it on a small tablet that was in a specific view?
It’s a fact this works with v5. I still recommend upgrading.

There is no other methods to try that I know of. V5 you can use the formatting tags.
