Customer Create Account button greyed out

On the Customer Edit screen (after a customer has been created) the “Create Account” button is greyed out.

How do I activate this button? The following is from previous posts about this …

In Manage->Accounts->Accounts, do you see the Customer listed under Customer Accounts ? … No

Also you need to set the Customer Account to Credit
Manage->Accounts->Account Types->Customer Accounts->Rules - Set to Credit … Yes

You need to fill in all customer fields based on your Account Template.

For example, with the Template shown below, you would need to fill in the Name and Phone before you will be allowed to create an Account.
